Now that you've been accepted, we need you to read and sign our Code-of-Conduct and Waiver forms, and fill us in on some personal info for your All Star page and to receive your product and merchandise.

When you submit the Code-of-Conduct form, you will be led straight to the Waiver form, and from the Waiver form to the Personal info form. We encourage you to fill out all the forms at once. You can also review them separately on the right of this page. We look forward to hearing from you.
